Safety system incidents are always unplanned and most unwelcome. At the same time, these events provide us with the opportunity to learn and prevent similar incidents in the future. Senior leaders have a unique role that we will highlight in a presentation at the Executive Health and Safety Council of British Columbia.
Jeff Coleman, the director responsible for incident investigations at Technical Safety BC will share the learnings from the inadvertent release of ammonia that killed everybody who was present in a mechanical room of an ice rink three years ago in Fernie, BC. This incident shows how safety issues can invisibly incubate in any organization that uses technical equipment. Based on the investigation, we have actively worked with many parties throughout the refrigeration industry and the recreational facilities industry to address the root causes.
